Advocate Family Support

This is the documentation process that should be followed for each family:

  1. REQUIRED:  Each family should have a Contact Us request filled out.  If this was a direct referral from someone, then the Advocate needs to fill out the Contact Us form for documentation and tracking purposes.
  2. EMAIL NOTIFICATION:  If a request comes in to the answering service, then an automatic email (or text if there is not an email) to the family contact.
  3. FAMILY WEBPAGE – If a family requests a page be set up then these two steps should occur:
    • Send them an email requesting the required information.
      • Example email for baby (no pictures)
      • Example email for person (with pictures)
    • Submit the information via the Family Webpage request.
    • If the family does not request a family page, then this email should be sent.
  4. REQUIRED:  A Family Close Out Form needs to be completed for EVERY family.
  5. OPTIONAL (discretion of the Lead Advocate):  At the conclusion of helping, a Family Survey should be completed for each family depending on the level of support.
